Make an Inpatient Referral

Drake Center specializes in helping patients who need more time to recover from catastrophic injury and illness. Often these are patients who have experienced traumatic brain injury, spinal cord injury or stroke, or are in need of advanced wound or pulmonary/ventilator care. Drake Center offers two levels of inpatient care - Long-term Acute Care (LTAC) and Transitional Skilled Nursing Care (TCU).

If you have a patient who could benefit from our extended rehab and medical care services, we offer a streamlined referral process to facilitate a smooth patient transfer:

To make a referral, call Drake Center at 513.418.4365 or 1.800.948.0003 x 4365, or complete the above online referral form.
Within 24 hours, one of our assessment nurses will provide a free patient evaluation to determine whether or not he/she meets the criteria for admission and also will speak with the family.
If the patient meets the admission criteria, our assessment nurse will contact the patient's insurance carrier to verify benefits, determine the need for additional assistance and obtain approval for a transfer to Drake Center.
Once approved, our assessment nurse will notify the referring facility's discharge planner and work with our admitting office to coordinate the transfer.
